FINANCIAL POLICY
Thank you for choosing Dr. Brian Howe as your dental health care provider. We are committed to your treatment being successful. All patients prior to seeing the dentist must read and sign our Health History, Insurance and Financial Policy Statement. The following is a Statement of our Financial Policy, which we require you to read and sign prior to any treatment in order to avoid any confusion of how we collect our fees for services rendered.
Dental Insurance
Our customary fees are the same for all patients whether or not they are insured. Each patient is financially responsible for his or her own account. As a courtesy to those patients with insurance, our office will be happy to file an electronic claim on your behalf. We ask you to be familiar with your policy benefits and limitations. Please be aware that insurance may vary greatly in types of coverage they offer. If within 60 days we have not received payment from your dental insurance, we will request the balance be paid in full. Any balance after payment from the insurance company will remain the patient’s responsibility. A treatment estimate is given at the time of diagnosis to give you an idea of what we ESTIMATE the insurance company will pay. It will list all services needed, an estimation of insurance benefit, and an estimated patient portion.
Cancellation policy
We are working to accommodate all patients in our schedule such that we will implement the following policy. Appointment cancellations require 24 hour advance telephone notice. Failure to provide notice will result in a $25 charge. Following any additional occurrences, we will require pre-payment of your portion, in full, prior to the appointment.
Payment
In keeping the highest level of professional care and maintaining our fees we have established the following payment options for our patients:
- Your ESTIMATED payment is due at the time of service. Visa, Mastercard, American Express and Discover cards are accepted payments.
- After payment from your insurance company, should there be any difference in the insurance company’s determination of usual and customary rates, it will be the responsibility of the patient to pay the remaining balance. We are using some automated billing and payment platforms to ensure safety for this process.
- We offer CareCredit, a dental financial plan, which allows qualified patients to make payments over a period of time. You may apply in the office or online for CareCredit. Please feel free to ask any questions you may have We thank you for your understanding and cooperation of our financial policies.
